Total Cost of Ownership

Also known as: TCO

The full multi-year cost of a purchase including implementation, integration, interfaces, training, support, internal staffing, and eventual replacement, not just the license or purchase price.

Total cost of ownership is the number a healthcare finance reviewer actually cares about. The quoted price is one line in it.

For clinical software the components that surprise buyers are consistent: interface build and maintenance, the internal analyst time to support the system, training for a workforce with turnover, the cost of the parallel process during rollout, and the governance and monitoring effort after go-live.

For AI products there is an additional and frequently unbudgeted category: ongoing performance monitoring, revalidation after model updates, and the clinical and informatics time that consumes.

A vendor who volunteers a realistic TCO model rather than waiting to be asked is unusual and is remembered. A vendor whose quoted price collapses under the first serious finance question is also remembered.
