Ambient Clinical Documentation
Also known as: Ambient AI Scribe, Ambient Documentation
Software that listens during a clinical encounter and creates a draft note for clinician review and signature.
Ambient clinical documentation is a workflow category rather than a diagnostic one. The product captures the conversation or another encounter audio stream, turns it into structured text, and prepares a note that the clinician reviews, corrects, and signs.
The buying review centers on note quality, remaining correction work, specialty coverage, what happens to audio and transcripts, EHR write-back, consent, support, and cost at enterprise scale. A fluent draft is not a finished medical record and does not remove clinician responsibility for the signed note.
Products sold under this label may also include coding support, order suggestions, or other functions that carry different evidence and governance requirements. Buyers should separate those functions instead of treating the ambient label as one risk category.
See the ambient clinical documentation guide and the ambient documentation vendor directory for the institutional buying layer.
